TOM offers two flat-rate, all-inclusive monthly memberships: Basic at $199/month(compounded semaglutide up to 2.4 mg per week) and Premium at $249/month(compounded tirzepatide up to 15 mg per week, or compounded semaglutide above 2.4 mg up to 7.2 mg per week). Your membership covers your prescribed medication, a personalized treatment plan, direct access to your board-certified obesity medicine physician, ongoing medical monitoring and dose adjustments, prescription management, and 24/7 access to medical guidance — all delivered to your door.
TOM doesn't take insurance, which is how we're able to keep pricing transparent and predictable for every patient. Your monthly membership covers everything — medication, physician access, and ongoing care — with no claims, copays, or surprise bills. We accept HSA and FSA payments.

Basic ($199/month) covers compounded semaglutide up to 2.4 mg per week, ideal for patients beginning GLP-1 therapy or looking for a steady, lower-cost option. Premium ($249/month) covers compounded tirzepatide up to 15 mg per week (or higher-dose compounded semaglutide up to 7.2 mg per week), for patients who may benefit from dual GLP-1 + GIP therapy or higher dosing. Both plans include the same level of physician-led care.

GLP-1 (glucagon-like peptide-1) is a natural hormone your body produces. GLP-1 medications are prescription medications that mimic that hormone to help regulate appetite, fullness, and blood sugar.
GLP-1 medications help you feel full sooner and longer, slow how quickly food leaves your stomach, and steady your blood sugar response, making it easier to eat less without feeling constantly hungry.
The most common side effects are gastrointestinal — nausea, vomiting, diarrhea, or constipation — and tend to ease as your body adjusts. Rare but more serious risks like pancreatitis or gallbladder issues are why ongoing physician oversight matters.
There isn't one "best" GLP-1; the right one depends on your medical history, tolerance, and goals, and your physician will guide that decision with you. In clinical studies, tirzepatide has shown greater average weight reduction than semaglutide , but the right choice is patient-specific.

The most common are gastrointestinal — nausea, vomiting, diarrhea, and constipation — usually strongest when starting or increasing a dose. Rarer serious risks include pancreatitis, gallbladder issues, and kidney concerns.
